CLCHC is striving to provide health care to Lake County, Lauderdale County and the surrounding counties for hope of better health for all residents. The facilities in Tiptonville and Ripley have an Outreach and Enrollment Counselor to assist with the Marketplace Insurance enrollment. The open enrollment for the Marketplace ended March 31, 2014, but it will open again in November of 2014. Our Outreach and Enrollment Counselors can assist with life changes through out the year for the Marketplace insurances and can assist with enrollment in TennCare. We accept most public and private insurances. We also have a sliding fee scale for the uninsured that qualify.

Certified Application Counselors (CAC) & Designated Organization (CACDO) related to Affordable Care Act
CLCHC has signed an Agreement with the Centers for Medicare & Medicaid Services to be a Certified Application Counselor Designated
Counselors (CACs). These CACs have completed required training and may assist consumers, applicants, and individuals who are
interested to finding out about and possibly applying for the State's Federally-Facilitated Health Insurance Marketplace or Exchange.
The organization shall abide by the standards and requirements pursuant to 45 CFR 155.225, and shall abide by the applicable Privacy
and Security Standards 45 CFR 155.260.
