Federal Tort Claims Act  Medical  Malpractice  Coverage

This health center is a Health Center Program grantee under 42 U.S.C 254b, and a deemed Public Health Service employee under 42 U.S.C 233(g)-(n). This health center receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health related claims, including medical malpractice claims, for itself and its covered individuals.
Certified Application Counselors (CAC) & Designated Organization (CACDO) related to Affordable Care Act
CLCHC has signed an Agreement with the Centers for Medicare & Medicaid Services to be a Certified Application Counselor Designated
Organization (CACDO).

Counselors (CACs). These CACs have completed required training and may assist consumers, applicants, and individuals who are
interested to finding out about and possibly applying for the State's Federally-Facilitated Health Insurance Marketplace or Exchange.
The organization shall abide by the standards and requirements pursuant to 45 CFR 155.225, and shall abide by the applicable Privacy
and Security Standards 45 CFR  155.260.

Each physician is certified by the American Board of Family Medicine and comes to our practice with years of experience in their area of specialty.  Our board-certified nurse practitioners always consult with the attending physician prior to working with each patient in order to provide informed, consistent care at all times.

​Citizens of Lake County for Health Care Inc. with its four sites complies with Title VI of the Civil Rights Act of 1964 (42 U.S.C. 200d et seq.), which states that "no person in the United States shall, on the ground of race, color, or national origin, be excluded from participation in, be denied the benefits of, or be subjected to discrimination under any program or activity receiving Federal financial assistance."  Any person who believes that discrimination has occurred by a state agency on the basis of race, color, or national origin, including limited English proficiency (LEP), in violation of Title VI may file a written complaint to the Title VI Coordinator, 710 Carl Perkins Parkway, Tiptonville, TN 38079 or call 731-253-6690.  Complaints must be filed within 180 days of the alleged discriminatory act.

What is an FQHC?

Federally Qualified Health Centers are community-based health care providers that receive funds from the HRSA Health Center Program to provide primary care services in underserved areas. They must meet a stringent set of requirements, including providing care on a sliding fee scale based on ability to pay and operating under a governing board that includes patients.

Federally Qualified Health Centers may be Community Health Centers, Migrant Health Centers, Health Care for the Homeless, and Health Centers for Residents of Public Housing.
